About the Role

An exciting opportunity is available for an experienced Support at Home Care Partner to join our team in Wagga Wagga. This role is ideal for someone who is passionate about helping older Australians remain independent and living safely in their own homes.

Working closely with participants, families, support workers and health professionals, you will coordinate services, monitor care delivery, and ensure participants receive high-quality, person-centred support.

Key Responsibilities

· Coordinate and manage a portfolio of Support at Home participants.

· Undertake participant assessments and reviews.

· Develop, implement and monitor individual support plans.

· Liaise with participants, families, support staff and external service providers.

· Ensure services are delivered in accordance with participant goals, funding requirements and organisational policies.

· Monitor participant wellbeing and identify changing care needs.

· Maintain accurate participant records and documentation.

· Support compliance with aged care quality standards and relevant legislation.

About You

To be successful in this role, you will have:

Essential

· Previous experience in Care Coordination, Case Management or a similar aged care or disability role.

· Strong understanding of home care services and the aged care sector.

· Excellent communication and relationship-building skills.

· Ability to manage competing priorities and work independently.

· Strong computer and administration skills.

Desirable

· Qualification in Nursing, Allied Health, Community Services, Ageing Support or a related field.

· Current driver's licence.
