Aboriginal Health Practitioner – Chronic Conditions

The Aboriginal Health Practitioner (AHP) is a vital member of Wunan Health’s’ multidisciplinary team (MDT), providing culturally safe, person-centred care for individuals and families.

Key Responsibilities:

Clinical Care

• Support GP and practice operations by providing high-quality, patient-centred care within scope of practice and in line with best available evidence 
• Duties include triage, immunisations and injections, wound care, ECGs, pathology collection, health assessments, chronic disease monitoring and support, and assisting medical practitioners with procedures. This includes spirometry and basic audiometry, with training and competency support provided. 
• Deliver routine primary care services including preventative screening, patient monitoring, follow-up, and care for acute, chronic and complex conditions. 
• Communicate clearly with patients, providing explanation, reassurance, support and advocacy throughout care. 
• Maintain accurate, timely clinical documentation and ensure all clinical activity meets professional, practice and accreditation standards. 
• Work collaboratively with GPs, nurses, allied health and the broader multidisciplinary team to support coordinated, culturally safe and holistic care. 

Patient Services and Care Coordination

• Support triage, care planning, diagnostics, data management, patient advocacy and coordination across internal and external providers. Assist patients to navigate the health system and access appropriate services. 
• Coordinate recalls, follow-up and outreach, including preventative health programs, GP Management Plans, Team Care Arrangements and other care planning activities. Communicate test results and follow-up information as directed by medical staff. 
• Participate in multidisciplinary meetings and case conferencing to support integrated and continuous care. 

Health Promotion and Community Engagement

• Support preventative health screening and patient education to improve health literacy, encourage healthy behaviours and support self-management. 
• Participate in outreach clinics, home visits, hospital visits and community-based care as required, including antenatal and postnatal support where relevant. 

Equipment, Infection Control and Supplies

• Ensure safe use, sterilisation and maintenance of clinical equipment. Manage clinical stock including storage, rotation, refrigeration monitoring, expiry management and ordering of supplies. 
• Maintain infection prevention and control standards and support staff education in safe clinical practice. 

Compliance and Professional Practice

• Maintain awareness of relevant legislation, accreditation standards and evidence-based practice in general practice. 
• Comply with infection control, sterilisation, hazardous materials handling, records management, OHS requirements and clinical waste disposal. 
• Engage in continuous quality improvement and contribute to system and process improvements. 
• Practice within scope, professional codes of ethics and competency. Maintain ongoing professional development and support education of staff where appropriate. 

General Responsibilities

• Support general practice operations and undertake other duties within scope as required. 
• Comply with workplace health and safety requirements and contribute to a safe environment for patients, staff and visitors.
