About the role
Key Responsibilities
· Coordinate day-to-day SIL home operations, including rosters, staffing coverage, leave and oncall requirements.
· Support participant onboarding, transitions, house compatibility and assessment of support
needs in consultation with participants, families, guardians, Support Coordinators and other
stakeholders.
· Implement, monitor and review individual support/care plans, goals, risk assessments,
behaviour support strategies and emergency plans.
· Ensure staff deliver supports in accordance with participant plans, risk/behaviour strategies
and organisational procedures.
· Support participants with daily living, independence, community participation and
achievement of personal goals.
· Monitor participant wellbeing, behaviour, health, safety and safeguarding concerns and ensure
appropriate escalation and follow-up.
· Promote participant rights, dignity, choice, privacy, safety and independence.
· Coordinate with health and allied health professionals to support participant outcomes and
continuity of care.
· Conduct regular SIL house audits, inspections and spot checks and address identified issues.
· Ensure participant documentation, care plans, risk assessments, emergency plans and other
records remain current and audit-ready.
· Review timesheets and monitor completion of shift notes, progress notes, medication records
and other required documentation.
· Support staff onboarding, orientation, mentoring, performance development and training.
· Liaise effectively with participants, families, guardians, Support Coordinators, allied health
professionals and other stakeholders.
· Support incident, complaint and safeguarding management, including reporting, escalation,
investigation and follow-up.
· Contribute to NDIS compliance, audit preparation, quality assurance and continuous
improvement.
