Undertake home visits, either alongside a Care Partner or independently as directed, to support administrative tasks, client documentation and routine client communication.
Ensure all work is completed in accordance with organisational policies, procedures and Support at Home program requirements.
Raise and document incidents, hazards and client feedback within organisational systems, ensuring timely escalation where required.
