About the role
Passionate about disability support, but ready for a role where you shape the plan, not just follow it?
If you love working alongside people living with disability, but you’re ready for more autonomy, more connection and the chance to genuinely drive the care you deliver, this could be the opportunity you’ve been waiting for.
At Leora, our Care Partner role offers a refreshing shift in pace and purpose. This is a dynamic, community-based role where you’ll support NDIS participants in their homes and local communities through thoughtful assessment, person-centred planning and hands-on coordination — making a meaningful impact in people’s everyday lives.
About Leora
Our name, Leora, means light and that’s exactly what we aim to bring: clarity, confidence and care when it matters most.
Leora Healthcare is a multi-award-winning provider delivering disability, aged care and community-based services across Australia. We support people to live with greater independence, wellbeing and choice through high-quality, person-centred care tailored to their individual needs.
We are proud to be recognised not only for the quality of our care, but also for the culture we have built. Leora has been named an Australian Financial Review Best Place to Work for five consecutive years (2021–2025), an HRD Employer of Choice (2023), a Top 100 Global Inspiring Workplace (2023).
Everything we do is guided by our values: Clients First, Masterful Simplicity, Unleashing Potential, and Do the Right Thing.
About the Role
We’re looking for an experienced and compassionate Care Partner to join us, working across home and community settings to support our growth in the Southeast region, Sydney.
This is a dynamic, field-based role – not a desk job. You’ll be out in the community, supporting clients in their homes and local settings, delivering high-quality, person-centred coordination and care.
Your portfolio will focus on clients funded under the NDIS, including people living with disability and those recovering from injury under iCare NSW ensuring support is personalised, respectful and aligned with funding requirements.
