About the Role:
MMT Recruitment is seeking a dedicated Social Worker on behalf of a highly respected, community-based healthcare provider in North-West Tasmania. In this vital role, you’ll support individuals experiencing severe and complex mental health challenges through person-centred case management and care planning.
This is a non-outreach, office-based and community-facing position within a multidisciplinary mental health team. You'll have the opportunity to build strong therapeutic relationships and provide coordinated support that empowers clients to improve their mental wellbeing and navigate life more confidently.
Key Responsibilities:
Conduct clinical assessments and develop personalised care plans
Coordinate with GPs, psychiatrists, psychologists, and allied health professionals
Support client advocacy during medical and social appointments
Engage with family and carers where appropriate
Maintain clear documentation and participate in team meetings
Contribute to health promotion, community engagement, and recovery-oriented care
Requirements:
Degree in Social Work or related discipline
2–5 years’ experience in community mental health or social support roles
Strong understanding of psychosocial support principles and trauma-informed care
Excellent communication and stakeholder engagement skills
WWVP check and valid driver’s licence essential
Must have full Australian work rights
