About the role
We are hiring FEMALE workers who have a passion in working alongside participants under NDIS to build capacity and promote autonomy in meeting participants goals in their daily living, with our expanding our supported independent living profile.
Supports required are for participants in the northern suburbs including Smithfield, Andrews Farm and Salisbury North. Workers are required to have FULL Australian car License.

Position Purpose The purpose of this position description is to outline the roles and responsibilities as a disability support worker to assist our participants to complete tasks, build their capacity and engage them in with the community.

KEY ACCOUNTABILITIES AND RESPONSIBILITIES
● Provide personal care, such as showering, dressing or other duties, responding flexibly to participant preferences.
● Comply with the NDIS Code of Conduct to support participants in ways that are respectful and inclusive.
● Apply principles of positive behavior support when supporting participants to implement their behavioral support plan.
● Assist with community access including but not limited to assisting participants in shopping, social events, including attending medical and other appointments.
● Ability to work independently and within a team environment.
● Provide services to the participants as per the Participant’s support plan developed by Client Services Manager in conjunction with participant.
● Provide services to the participants as per the Participant’s Health Care Plan developed by a qualified Registered Nurse.
● Must comply with health care need training in line with the developed Health Care Plan.
● Must maintain Participant confidentiality at all times.
● Participate and attend to and comply with all NDIS training requirements.
● Ensure progress notes and other documentation are clearly documented and signed at each shift.
● Comply with Holistic Care SA policies and procedures.
● Must comply with the Work Health and Safety (WHS) Principles including safe manual handling procedures.
ESSENTIAL MINIMUM REQUIREMENTS
● Certificate III in Individual Support (Ageing or Disability) qualification
● Current NDIS Work Worker Check
● Current First Aid Certificate
● Current CPR Certificate
● NDIS Worker Orientation Module
● Current Manual Handling Certificate
● Mandated Notification Training and Working with Children’s (WWC) Check (if working with children)
DESIRABLE REQUIREMENTS
· Current Medication Administration
· Current Infection Control
· Current Continence Care Management
STATEMENT OF KEY OUTCOMES AND ASSOCIATED ACTIVITIES
The performance of the disability support worker will be appraised by the Client Services Manager and Client Services Officer. The key areas of evaluation will be:
Participant focus:
Meet the participant's needs in a timely and professional manner.
● Support the participant to achieve their goals.
● Report any concerns you may have noticed during the shifts.
● Actively engage and interact with the participants activities/daily living requirements.
Communication and teamwork
Effectively and actively listens and observes, conveying and receiving information and willingness to accept and take direction from participants and office staff.
● Flexibility with participant needs and requirements, with willingness to accept extra shifts often with short notice.
● Well written and verbal communication with the participants and office staff.
● Work harmoniously and respectfully with all staff, demonstrating support for other team members.
● Willingness to work as part of a team to achieve service excellence.
● Develop a sound and clear understanding of the services delivered by Holistic Care SA.
● Ensure all communication and teamwork aligns with Holistic Care SA Pty Ltd Core Values.
Professionalism
Represents Holistic Care SA in a professional manner and promotes a positive image of Holistic Care SA to participants, other services providers and the community.

Time management
Submit and complete documentation/reports in a timely manner as per Holistic Care SA Pty Ltd Policy and Procedures.
● Attend designated shifts as per roster.
● Advise the rostering team of changes of availability in a timely manner.
● Ensure availabilities are updated in a timely manner as per contract requirements.
KEEP SAFE WORKING PRACTICE BY:
Carry out all duties with diligence and due care for personal safety and safety of others
● Report all incidents, accidents, near misses injuries and hazards promptly
● Notify Holistic Care SA immediately in the event of any equipment failure.
● Be vigilant in monitoring individual work practices
● Report any concerns with respects to health and safety to Holistic Care SA
SUPPORT WORKER DUTIES
The role of the Disability support worker varies depending on the participants’ support needs and services requirement, however general duties that are expected of the position including:
1. Assistance in personal care including feeding, toileting, bathing and grooming, dressing/undressing, hair routine, dental care and where applicable makeup application.
2. Medication – administering medication or other medical/special procedures, as outlined in the individuals Health Care Plan. Disability support workers must not administer medication unless completion of relevant medication training and follow as per the support plan.
Transporting participants in vehicles to various locations (such as shopping, attending appointments and social events) or assistance using public transport.
Provide domestic assistance, including but not limited to:
11. Where required disability support workers will provide overnight support in the Supported Independent Living (SIL) Homes, this service involves staying overnight in the participant’s home and may include any or all aspects of personal and home care services.
● Undertake compulsory WHS Training
● Participate in WHS training and development programs and consultative processes, following any incident occurring.
● Identifying any potential hazards and reporting them to Holistic Care SA, in line with incident reporting and WHS training guidelines and policy and procedures.
PERSONAL ABILITIES/APTITUDES/SKILLS
● A passion in providing support to people with disability where the whole person is supported enabling them and equipping them to live an independent lifestyle.
● Demonstrated capacity to work with honesty and integrity.
● Ability to communicate effectively and appropriately with people with disabilities and their families and taking into account age and culture
● Ability to respond appropriately to individual needs and priorities
● Work with limited supervision as well as working as part of a team
● Demonstrate good organizational skills
● Practical written and verbal communication skills.
● An ability to provide a range of personal care and domestic assistance to participants in a way that promotes their independence, values, rights and dignity
● A reasonable knowledge and understanding of the NDIS Code of Conduct, National Disability standard, WHS, Equal Opportunity Principles and Practices and knowledge of standard safe food handling procedures.
