Neurological Physiotherapist - Grade 2 (Rehab at Home)

About the role:

An opportunity exists for an enthusiastic Physiotherapist with Neurological skills to join our collaborative and dynamic Rehab at Home team. Rehab at Home operates within the Division of Western Health at Home. Rehab at Home is a subacute, bed substitution program that provides intensive, short-term rehabilitation to patients discharged early from hospital.

Key responsibilities:

• Provision of intensive rehabilitation for clients who have been discharged early from hospital to sustain discharge and prevent representation to hospital.
• Provision of goal-directed therapy in clients' homes, in the centre, or via telehealth (Health Direct).
• Collaborating with the interdisciplinary team to achieve client directed goals.

About you:

• Experience in assessing and managing clients with a variety of neurological conditions.
• Excellent organisation and time management skills.
• Experience supervising Grade 1 Physiotherapists, Physiotherapy students and/or Allied Health Assistants.
• Demonstrated experience in quality improvement and/or research.
