Disability Support Worker - High Intensity Supports

·       Deliver safe, compassionate and person-centred support in accordance with each participant's goals, preferences and support plan.

·       Assist with personal care, hygiene, dressing, toileting, mobility, transfers, meals and other daily living tasks.

·       Support participants to build independence, maintain routines and access community, social and civic activities.

·       Provide high-intensity and complex supports within your training, competence and delegated responsibilities.

·       Follow positive behaviour support plans, risk controls, health plans and incident response procedures.

·       Provide medication assistance in accordance with policy, training and participant documentation.

·       Observe and report changes in a participant's health, wellbeing, behaviour or support needs and escalate concerns promptly.

·       Maintain accurate, timely and respectful progress notes, medication records, incident reports and other required documentation.

·       Work collaboratively with participants, families, support coordinators, healthcare professionals and colleagues.

·       Protect participant privacy, dignity, choice, cultural identity and rights while maintaining professional boundaries.

·       Follow NDIS Practice Standards, work health and safety requirements, infection control procedures and organisational policies.
