About the role
Videri is a professional, innovative and fast growing company in the aged-care sector.
We operate small group homes in the Eastern Suburbs, Hunters Hill, Hornsby Shire, North Shore, Northern Beaches, Hills District, Inner West, and Sutherland Shire that are warm, luxurious and friendly, catering to a maximum of just 10 residents per home.
We support our residents to flourish, keeping them part of their local communities and engaging with them in a purposeful and meaningful way. We encourage them to be involved in the day to day running of the homes including cooking, gardening and light housework. It is a truly unique, wonderful and inviting environment in which to work. Our social model is underpinned by a high standard of clinical care and case management.
About the RoleWe are seeking a compassionate and experienced Social Worker/Counsellor to join our team across multiple locations in Sydney. In this role, you’ll provide psychosocial support to residents and families, help guide smooth transitions, and build strong collaborative relationships across our home teams.
You’ll be a key link between residents, families, Registered Nurses, Homemakers, and other professionals - ensuring people feel supported, understood, and empowered throughout every stage of the dementia journey.
What You’ll Be DoingBuild warm, consistent, and proactive relationships with families through regular check‑ins and support conversations.
Provide guidance through sensitive transitions and end‑of‑life moments.
Facilitate debrief sessions with Homemakers to support resilience and wellbeing.
Work closely with Registered Nurses, Neighbourhood Managers, and Homemakers to integrate psychosocial needs into daily care.
Participate in weekly huddles and Resident Care Meetings, ensuring follow‑up and continuity of care.
Partner with the Engagement Team during assessments, admissions, arrivals, and departures.
Complete psychosocial assessments to understand life stories, family dynamics, and resident needs to support appropriate placements.
Ensure assessments remain living documents informing daily care.
Contact families before admission to build confidence and ease transitions.
Brief teams to prepare for new residents and complete follow-up check‑ins post-arrival.
Champion a warm, dignified welcome for every resident.
Provide families with education on dementia progression, grief, and coping.
Identify early signs of carer stress and link families to relevant supports.
Advocate strongly to ensure resident and family voices guide decisions.
Bachelor of Social Work / Bachelor of Counselling
Alignment with Videri’s values and philosophy
Comfort and affinity working with older adults and people living with dementia
Excellent interpersonal and active listening skills
Strong teamwork and collaboration capability
Valid driver’s license + access to personal vehicle
Solid computer skills
Experience supporting people living with dementia
Australian working rights
NDIS Worker Screening
Willingness to work within the Aged Care and NDIS regulatory framework
Some out‑of‑hours or weekend work may be required
