Position Title: Direct Service Facilitator (NDIS & Aged Care)
Location: Office-based in Ipswich, with client base across West Moreton
Employment Type: 3-Month Contract – Immediate Start
We are seeking a highly motivated Direct Service Facilitator to join our Ipswich team, working across both NDIS and Aged Care Services. This is a 3-month period role with an immediate start. The successful candidate will play a key role in onboarding new participants and facilitating support services to meet the needs of our NDIS and Aged Care clients.
You will be an excellent communicator and team player, sharing an office environment with 5 colleagues, and will bring a strong understanding of NDIS funding and reporting requirements to ensure compliance and high-quality service delivery.
Key ResponsibilitiesFacilitate the onboarding process for NDIS and Aged Care participants.
Work collaboratively with colleagues, participants, families, and service providers to ensure support needs are met.
Maintain accurate documentation and reporting in line with NDIS and Aged Care requirements.
Provide guidance to participants on service options and ensure their goals and outcomes are supported.
Assist with coordination and communication between participants, support workers, and relevant stakeholders.
Contribute to a positive and professional office culture, supporting team objectives.
Minimum 5 years’ experience working in NDIS, Aged Care, or related community services.
Strong knowledge of NDIS funding models, compliance, and reporting requirements.
Demonstrated experience in participant onboarding and service facilitation.
Excellent communication, problem-solving, and interpersonal skills.
Ability to work both independently and as part of a collaborative office team.
High-level organisational skills with attention to detail.
Minimum Certificate IV in Community Services (or Equivalent)
Current or eligibility to obtain:
Blue Card (Working with Children Check)
Yellow Card / NDIS Worker Screening Check
National Police Check
Open Driver’s License with clear history to drive ALARA Pool vehicles .
