Care Partner for Support at Home provider

Are you an Allied Health or Registered Nurse professional who is ready to change career paths and join a not-for-profit organisation which specialises in the delivery of Home Care Support across the Eastern Suburbs area Sydney. Draw on your previous experience working with patients within a clinical setting and adapt those skills to our community clients. 

Responsibilities include:

• Proven experience working with complex clients, needs assessments and risk/safety assessments

• Create and maintain the client’s case management plan including conducting assessments, and determining the services needed in collaboration with the client.

• Advocate on behalf of our client to a variety of organisation and ensure that all services are deliverable against their government funding

• Develop, implement, monitor and review case plans in collaboration with the client and families

• Provide comprehensive assessments to identify client needs and requirements for clients using internal and external service networks.

• Following up on leads that are generated through word of mouth, marketing and other initiatives.

To be successful you must: 

• Have experience and an understanding of Support at Home Program

• Have a tertiary qualification in either Nursing, Allied Health, Disability or Gerontology

• Be organised and have strong attention to detail

• Strong written and verbal communication

• Customer focused attitude and a passion for making a difference with the vulnerable and elderly.
