Derbarl Yerrigan Health Service is seeking a compassionate and motivated Aboriginal Health Practitioner (AHP) to join our dedicated Maternal & Child Health Team. This is an exciting opportunity to work alongside Child Health Nurses, GPs and multidisciplinary health professionals to support Aboriginal children and families during some of life’s most important stages.

About the role:

As an Aboriginal Health Practitioner within the Maternal & Child Health Team, you will play a vital role in supporting Aboriginal children, parents and families across the Perth metropolitan area.

You will provide culturally secure care, health education, advocacy and care coordination, helping families access the health services and support they need to achieve positive health outcomes.

Working closely with Child Health Nurses and other health professionals, you will help strengthen engagement with preventative health services, coordinate appointments, support follow-up care and promote healthy families and communities.

Responsibilities:

• Providing culturally safe care and support to Aboriginal children and families.
• Coordinating appointments and assisting families to navigate healthcare services.
• Supporting maternal and child health programs and follow-up activities.
• Undertaking health screening activities, including observations and vital signs.
• Assisting with immunisation programs and recalls.
• Delivering health education and promoting healthy lifestyles.
• Advocating for clients and supporting access to hospital and community-based services.
• Building strong relationships with families, community organisations and healthcare providers.
• Contributing to case management and multidisciplinary care planning.
• Maintaining accurate clinical records and documentation.

What we’re looking for:

• Current Registration with the Australian Health Practitioner Regulation Agency as an Aboriginal Health practitioner or Certificate IV in Aboriginal and/or Torres Strait Islander Primary Health Care Practice or equivalent qualification from relevant industry/profession or working towards Certificate IV in Primary Health.
• Experience working with Aboriginal communities in a healthcare setting.
• Strong understanding of Aboriginal culture and health issues.
• Excellent communication and interpersonal skills.
• Ability to work collaboratively within a multidisciplinary team.

Other Requirements:

• Aboriginal and/or Torres Strait Islander pursuant to Section 50D of the Equal Employment Opportunities Act (Essential).
• Current Working with Children Check.
• Current National Police Clearance
• Valid Drivers’ License and willingness to travel as required.
• Commitment to ongoing professional development and training.

Desirable:

• Experience working in maternal, child or family health.
• Experience using Communicare, AIR and Medicare systems.
• Immunisation qualifications or willingness to obtain.
