About the role
Your new role:
As an Aboriginal and Torres Strait Islander Health Worker – Chronic Disease, you will play a key role in improving health outcomes for Aboriginal and Torres Strait Islander people and families across the Logan community. Working within a multidisciplinary team, you will provide culturally appropriate chronic disease support, health education and care coordination while building strong connections between clients, families, community and health services.

You're a culturally knowledgeable and community-focused health professional who can build trusted relationships and work confidently with Aboriginal and Torres Strait Islander people, families and multidisciplinary teams.
