About the role
Become part of our Aged Care Assessment team, based at The Royal Melbourne Hospital (RMH), Royal Park campus. As part of the single assessment system workforce across Melbourne's Northern and Western Metropolitan regions, you'll support older Australians to access Commonwealth-funded aged care programs and services that meet their individual needs.
As a Clinical Aged Care Assessor, you'll undertake strengths-based assessments with older Australians across hospital, community, home and telehealth settings. You'll identify their goals and support needs, develop care plans, complete reviews and make referrals to help them access the right care and services.
