Lifestyle Support Worker – Clermont- Short Term

Stream Services is seeking compassionate and experienced Lifestyle Support Workers to join our growing team for a 1-month short-term contract based in Clermont, QLD. This rewarding role supports people with disability to live independently, stay connected with their community, and achieve meaningful personal goals. 

This is an excellent opportunity for support workers based in Brisbane, Townsville, or surrounding areas who are interested in short-term regional work. Accommodation and transport will be provided, and an immediate start is available for suitable applicants. Ideally, you will have experience in the disability sector and are looking to work within a supportive, positive team environment while making a genuine difference in the lives of others. 

What You’ll Do 

• Support NDIS Participants with daily living activities, including personal care, meal preparation and household tasks. 
• Provide social and recreational support to build confidence, independence and community participation. 
• Follow individualised support plans aligned with participant goals. 
• Ensure participant safety, wellbeing and dignity at all times. 
• Maintain accurate, NDIS-compliant documentation and reporting. 
• Work collaboratively with the wider team to deliver person-centred, high-quality care. 
• Be willing to provide overnight active support shifts where required to meet participant needs. 

About You 

We’d love to hear from you if you have: 

• Qualifications in disability support, aged care, community services, or equivalent experience. 
• Experience providing person-centred support to NDIS participants or aged care clients (preferred but not essential). 
• A warm, empathetic and professional approach. 
• Strong communication skills and the ability to build positive relationships. 
• Commitment to upholding the rights and dignity of people with disability. 
• Ability to work independently and as part of a supportive team. 
• Current First Aid and CPR Certificates. 
• Current NDIS Worker Screening Check. 
• Valid driver’s licence. 
• Full working rights in Australia.
