At Right at Home Ipswich, Lockyer Valley, we’re looking for a Full-Time Care Co-Ordinator to join our friendly and dedicated team, supporting clients across aged care and disability services in the community.

About the Role

As a Care Partner, you’ll be the vital link between our clients, their families, and our care team. You’ll ensure that every client receives personalised, high-quality support tailored to their unique needs, helping them live safely and independently at home.

Key Responsibilities

• Coordinate and schedule Support at Home services

• Match clients with suitable care professionals based on skills and compatibility

• Maintain regular communication with clients, families, and staff

• Monitor and update client care plans and quarterly budgets

• Ensure compliance with NDIS and Aged Care Quality Standards

• Deliver exceptional customer service and maintain accurate records

About You

• Minimum 1 year of experience in a similar Care Partner or Co-Ordination Role

• Sound understanding of NDIS and Aged Care Quality Standards (preferred)

• Strong organisational, time management, and communication skills

• Empathetic, client-focused approach to care

• Confident using scheduling software and IT systems

• Relevant qualifications in:

  • Certificate III or IV in Individual Support (Ageing or Disability), or

  • Diploma in Community Services
