Community Service Care Partner

In this role, you will:

• Build relationships with clients, understand their needs and goals, and act as their key contact.
• Coordinate and monitor services to ensure clients receive appropriate, person-centred support.
• Monitor changing needs, funding and safety, and arrange reviews or escalate concerns when required.
• Maintain accurate records and ensure services meet Support at Home and organisational requirements.
• Work with clients, families, staff and providers to support positive outcomes.

Full- Time/ Flexible Monday - Friday

Essential Criteria:

• Demonstrated success in a previous Care Partner or similar role 
• Working knowledge of the Support at Home program
• Exceptional people skills
• Passionate about going above and beyond for others
• Outstanding oral and written communication skills
• Exceptional organisational skills
• Ability to multitask and adapt to any situation
• Self-motivated and proactive
• First aid certificate (current or willing to obtain)

Desirable Criteria:

• Certificate III in Individual Support
