About The Role
Become part of our Aged Care Assessment team, based at The Royal Melbourne Hospital (RMH), Royal Park campus. As part of the single assessment system workforce across Melbourne’s Northern and Western Metropolitan regions, you’ll support older Australians to access Commonwealth-funded aged care programs and services that meet their individual needs.
As a Clinical Aged Care Assessor, you’ll undertake strengths-based assessments with older Australians across hospital, community, home and telehealth settings. You’ll identify their goals and support needs, develop care plans, complete reviews and make referrals to help them access the right care and services.
Key Accountabilities
Conduct assessments of older individuals to determine their care needs, preferences, and eligibility for aged care services.Work with clients, families, and care providers to develop personalized care plans that support the individual's physical, emotional, and social needs.Collaborate with other health professionals, government agencies, and service providers to coordinate services and ensure holistic care.Advocate for the rights of older adults and ensure their voices are heard throughout the assessment and care process.Monitor and review care plans to ensure they remain relevant and effective in meeting the changing needs of clients.
About You
You are an experienced Allied Health, Nursing or related professional who offers:
Minimum 12 months’ experience working with older people in aged care or a relevant clinical setting.Strong clinical assessment, communication and person-centred care skills.Knowledge of aged care services, policies and regulations.Ability to work independently and collaboratively within a multidisciplinary team.Strong digital skills and experience working to KPIs and quality outcomes.Current Australian driver’s licence and ability to travel for assessments.Commitment to The Melbourne Way – putting people first, leading with kindness and achieving excellence together.
