About the role
Premier Supported Living Group is seeking an experienced, organised and highly motivated Support Coordinator / Case Manager to join our growing team.
This role is ideal for someone who enjoys working closely with NDIS participants, families, hospitals, allied health professionals and service providers to coordinate supports, resolve barriers and help participants achieve meaningful outcomes.
You will manage a portfolio of participants with varying levels of complexity, including people transitioning from hospital into the community, participants requiring Supported Independent Living (SIL), and individuals who require strong coordination across multiple services and stakeholders.
About the Role
As a Support Coordinator / Case Manager, you will take responsibility for coordinating and overseeing participant supports from referral and onboarding through to ongoing service delivery and review.
You will act as a key point of contact between participants, families, Support Coordinators, hospitals, allied health professionals, NDIS stakeholders and internal operational teams.
The role requires someone who can confidently manage complex situations, communicate professionally and ensure participants have appropriate supports in place.
Key Responsibilities
· Manage a caseload of NDIS participants and oversee their support requirements.
· Build strong relationships with participants, families, nominees and key stakeholders.
· Coordinate participant transitions from hospital into community-based accommodation and support.
· Assist participants to understand and implement their NDIS plans.
· Coordinate SIL, community access, allied health and other funded supports.
· Work closely with hospitals, discharge planners, social workers, Support Coordinators and allied health professionals.
· Identify gaps in participant supports and coordinate appropriate services.
· Attend care-team meetings, case conferences and participant reviews.
· Monitor participant goals, progress, risks and changing support needs.
· Assist with preparation for NDIS plan reassessments and reviews.
· Coordinate supporting documentation from occupational therapists, behaviour support practitioners, physiotherapists and other clinicians.
· Support participants with accommodation and long-term housing pathways where required.
· Maintain accurate participant files, case notes, risk information and documentation.
· Follow up outstanding actions and ensure agreed recommendations are implemented.
· Escalate concerns relating to participant safety, wellbeing or service delivery.
· Work collaboratively with Premier's operations, rostering, intake, compliance and management teams.
· Ensure services are delivered in accordance with the NDIS Practice Standards and organisational policies.
About You
We are looking for someone who is proactive, calm under pressure and comfortable managing multiple stakeholders and competing priorities. You will ideally have:
· Previous experience in Support Coordination, Specialist Support Coordination, Case Management, Disability Services, Community Services or a similar role.
· Strong understanding of the NDIS and participant funding.
· Experience supporting participants with complex needs.
· Knowledge of Supported Independent Living (SIL) and community-based disability supports.
· Experience working with hospitals, health services or discharge teams will be highly regarded.
· Strong written and verbal communication skills.
· Excellent organisational and follow-up skills.
· Ability to manage a caseload independently.
· Confidence communicating with families, clinicians, hospitals and government stakeholders.
· Strong problem-solving and negotiation skills.
· Ability to prepare professional case notes, reports and participant documentation.
· A participant-centred and outcomes-focused approach.

· Disability
· Allied Health
· Psychology
· Nursing
· Case Management
· Human Services
Relevant industry experience may also be considered in place of formal qualifications.
Mandatory Requirements
The successful applicant will be required to hold or obtain:
· NDIS Worker Screening Check
· Working with Children Check where applicable
· Current Victorian driver's licence
· National Police Check
· Current First Aid and CPR certification, where required
· Full Australian working rights
