About the role
Deliver, manage and advocate for high-quality psychosocial care in geriatric medicine, supporting complex patient needs across inpatient and community settings.
Employment Type: Temporary Full Time (up to 16th July 2027)
Position Classification: Social Worker Level 1/2
Remuneration: $79,419 – $113,301 per annum (pro rata)
Hours Per Week: 38
Location: Cumberland Hospital Campus - North Parramatta

Our team is committed to specialised rehabilitation for older demographic from early stages in hospital through to community reintegration.
You will be a part of:

What you’ll be doing
In this dynamic role, you will provide comprehensive social work services to patients, families and carers, applying your professional judgement to support complex clinical needs.
Your key responsibilities will involve:
Delivering psychosocial assessments, counselling, advocacy and crisis intervention.
Supporting patients and families navigating complex issues including ageing, loss and care planning.
Collaborating with multidisciplinary teams in ward rounds and care planning.
Contributing to service improvement, education and evidence-based practice.
Maintaining accurate documentation, data reporting and professional standards.

Essential Criteria:
Qualifications: Bachelor of Social Work
Registration: Eligibility for full practicing membership of the Australia Association of Social Work (AASW)
Selection Criteria:
Demonstrated understanding of issues associated with ageing, age related illnesses, rehabilitation, accommodation placements, guardianship and end of life care.
Unrestricted NSW Driver’s Licence and ability to participate in the After Hours Crisis On-Call Social Work Service.
A good knowledge of community resources and experience liaising with community organisations
Demonstrated clinical experience or understanding of grief and loss counselling, crisis intervention, discharge planning and working with complex family dynamics.
Demonstrated understanding of issues associated with working with culturally and Linguistically Diverse client group.
Demonstrated well-developed time management skills with excellent written and verbal communication, including report writing skills and high level computer literacy.
Demonstrated ability to work with minimal supervision and as part of a multidisciplinary team
Commitment to quality improvement and evidence based practice and a commitment to ongoing clinical and professional learning
