Join the Social Work Aged Care Team at Nepean Hospital and play a vital role in supporting patients' psychological, emotional and social well-being throughout their hospital journey.
As a Social Worker, you will provide high-quality social work services to patients and families, addressing the complex psychosocial impacts of illness, treatment and hospitalisation. Working as part of a multidisciplinary health team, you will deliver crisis intervention, psychosocial assessment, counselling, advocacy, case management, therapeutic and grief/loss counselling, as well as complex discharge planning. You will help patients navigate challenges, promote smooth transitions through the hospital treatment system, and support positive outcomes for both patients and the organisation.
