$61.93 - $64.83 per hour (dependent on health profession) + 12% super + Salary Packaging Benefits

 About the Role

The Diabetes Educator works in partnership with clients and their families/carers to improve health outcomes and quality of life by empowering people living with diabetes to recognise their strengths, aspirations and goals.

Working within a social model of health and chronic condition self-management framework, you will provide high-quality diabetes assessment, education, therapeutic management and links to support services for individuals and groups. You will collaborate closely with multidisciplinary teams and external stakeholders to address identified clinical, social and behavioural needs and deliver coordinated, person-centred care.

Key responsibilities include:

• Providing health education, therapeutic management and support for diabetes prevention and management.
• Facilitating client and carer skill development in the use of diabetes healthcare products and monitoring equipment.
• Developing, implementing and evaluating diabetes education and self-management programs.
• Coordinating diabetes clinics as required.
• Contributing to service planning, quality improvement activities and evidence-based practice initiatives.
• Maintaining accurate client documentation, reports and service data.
• Participating in professional development and student supervision activities as required.

 About You

You are a client-focused clinician who is passionate about supporting individuals and communities to achieve better health outcomes.

You will bring:

• A strong understanding of community health and the social determinants of health.
• Excellent communication, negotiation, organisational and time management skills.
• The ability to work autonomously while contributing effectively within a multidisciplinary team environment.
• The ability to build and maintain collaborative relationships across the health and community service sectors.
• Experience working with culturally and linguistically diverse communities and refugee populations will be highly regarded.

To Succeed, You Will Need

• Post Graduate qualification in Diabetes Education from a recognised university.
• Current practising registration with AHPRA or relevant professional governing body.
• Credentialed Diabetes Educator status, or be actively working towards credentialing through the Australian Diabetes Educators Association (ADEA) credentialing program.
• Minimum two years' experience, preferably within community health or a community-based setting.
• Demonstrated ability to work both independently and as part of a multidisciplinary team.
• Excellent communication, negotiation, organisational and time management skills.
• Ability to liaise effectively across the full spectrum of health and welfare services.
• Current Victorian Driver's Licence.

Desirable

• Experience managing insulin stabilisations.
