The Community Assessment and Response Proactive Team are recruiting a Part-Time Credentialled Diabetes Nurse Educator.
About the Role
There is an opportunity for a part time permanent position as a Credentialled Diabetes Nurse Educator working at Monash Health community sites including Cranbourne Community Hospital.
The Community Assessment and Response Proactive Team is a large multi-disciplinary team covering multiple community sites across campuses including Pakenham, Cranbourne Community Hospital, Springvale, Berwick and Dandenong Community Health and works closely with the Monash Health Diabetes Unit. Your primary focus within this role is to provide diabetes education to patients in the community setting and liaison with all members of our friendly and supportive multi-disciplinary team. Specifically, you will provide evidence-based, patient-centred, specialised care and education for patients in the community.
