Fractional Specialist - Hospital Without Walls (HWoW)

About The Role

We are seeking a Fractional Specialist Physician or General Practitioner to deliver specialist geriatric and complex care to patients across the Grampians region — both in hospital and in the community. Reporting to the Clinical Directors, Hospital Without Walls and working as a senior member of the medical team, you will provide leadership and clinical expertise in a team-based environment.

In This Role You Will

Provide specialist clinical services to older patients with complex care needs, including direct and remote (telehealth) assessment, complex management planning, prescribing and collaborative team decision-making.Lead and support the multidisciplinary team to achieve clearly documented patient and team goals, and coordinate care with nursing, allied health, diagnostic and community services.Supervise, teach and mentor junior medical staff allocated to HWoW and Geriatric Medicine.Work closely with clinicians across the region to develop and deliver effective community-based services.Contribute to education, collaborative strategic planning, policy and procedure development, quality improvement and patient consultation.Ensure high-quality, complete documentation of patient care and evidence-based practice always.

About You

To be successful in this role you will have:

MBBS or equivalent degree, with current medical registration in Australia.Fellowship of the Royal Australasian College of Physicians (FRACP) or the Royal Australian College of General Practitioners (FRACGP), or an equivalent qualification recognised by the Royal Colleges.Demonstrated experience and skills in the medical assessment and clinical management of complex older patients.Detailed knowledge of the investigation and management of acute and chronic medical disorders, and an understanding of the impact of illness, disability and ageing on patients and their families.The ability to lead a multidisciplinary team and operate effectively at a senior clinical level.Excellent written and verbal communication skills, and the ability to work effectively both independently and as part of a team.A commitment to quality improvement, ongoing professional development and evidence-based practice.
