The Community Mental Health/Core Team model of care states: The “Community Mental Health Team” (Core Team) is the mainstay of the community mental health system. Core Teams and provide a range of clinical services including intake, assessment, consultation, treatment ,and care coordination. Care coordination is the main component of this model of care and includes extended care coordination, targeted packages of care, and short term care coordination.
