We are seeking an experienced Registered Nurse (RN) to join our growing clinical team as a Clinical Support Nurse.
This is a fully remote opportunity focused on conducting initial patient consultations and clinical assessments across pain, mental health and sleep. You will play an important role in gathering comprehensive clinical information and supporting the treating GP throughout the patient's care journey.
About The Role
As a Clinical Support Nurse, you will conduct initial telehealth consultations with patients prior to their GP appointment.
Your role will involve completing structured clinical assessments across pain, mental health and sleep, taking a detailed patient history and identifying any relevant clinical risks or concerns.
You will then provide the treating GP with a clear and comprehensive clinical summary to support their consultation and clinical decision-making.
This is a fully remote, 3-month contract position, with the possibility of extension.
What You'll Own
Conduct initial telehealth consultations with patients.
Complete comprehensive pain assessments, including pain history, symptoms, severity, duration, previous treatments and impact on daily life.
Conduct mental health assessments, including relevant history, current symptoms, psychosocial factors and risk screening.
Conduct sleep assessments, including sleep patterns, sleep concerns, symptoms and relevant contributing factors.
Obtain and document detailed medical and medication histories.
Review current treatments and previous treatment approaches.
Identify relevant clinical risks, contraindications and escalation requirements.
Escalate any clinical concerns appropriately to the treating GP or relevant medical practitioner.
Prepare clear and accurate clinical summaries for GP review.
Ensure relevant clinical information is captured before the patient's GP consultation.
Provide appropriate patient education and support within your scope of practice.
Maintain accurate and timely clinical documentation.
Work collaboratively with GPs and the wider clinical team.
Deliver a professional, compassionate and patient-centered telehealth experience. Participate in quality improvement initiatives, clinical audits, and ongoing professional development activities.
What You'll Need
Current unrestricted AHPRA registration as a Registered Nurse (RN).
Minimum 2 years' post-registration clinical experience.
Strong clinical assessment, patient interviewing and documentation skills.
Proven experience conducting pain, mental health and sleep assessments.
Experience in chronic pain management, mental health, sleep, primary care, community nursing or telehealth.
Confidence conducting patient consultations independently via telehealth.
Strong ability to identify clinical risks and escalate appropriately.
Ability to work autonomously in a fully remote environment.
Comfortable using clinical software and telehealth platforms.
Strong organisational and time-management skills.
Excellent communication and rapport-building skills.
A genuine commitment to delivering high-quality, patient-centred care.
Highly Regarded
Previous experience working with veterans or veteran-focused healthcare services.
Experience with medicinal cannabis.
Experience working within a multidisciplinary healthcare team.
Understanding of DVA and veteran healthcare pathways.
