About the Role

Heal at Home is seeking a Registered Nurse or experienced Enrolled Nurse to join our friendly and supportive team in a casual, office-based position.

This role would suit a nurse who enjoys combining their clinical knowledge with coordination, communication and administration. You will work closely with our General Manager, Clinical Leads and community nursing team to support the smooth delivery of our services.

The position includes:

• Receiving and screening new nursing referrals
• Gathering relevant client, clinical and funding information
• Coordinating initial assessments and nursing visits
• Scheduling appointments and allocating appropriately experienced nurses
• Communicating with clients, families, support coordinators, aged care providers and other stakeholders
• Responding to telephone calls and emails
• Supporting nurses working in the community
• Following up service agreements, reports, care plans and other documentation
• Maintaining accurate client and referral records
• Assisting with general clinical administration
• Participating in a rostered on-call arrangement, where required
• Working closely with the General Manager and Clinical Leads regarding clinical concerns and service priorities

All duties will be performed within the nurse’s individual scope of practice and in accordance with AHPRA and NMBA professional requirements. Enrolled Nurses will work under the direction and supervision of a Registered Nurse.

About You

You will be:

• A Registered Nurse or experienced Enrolled Nurse with current AHPRA registration
• Experienced in community nursing, the NDIS, aged care or a similar healthcare environment
• Familiar with referral management, scheduling or clinical administration
• Organised, approachable and confident communicating with different stakeholders
• Able to manage several priorities while maintaining accuracy and attention to detail
• Comfortable working independently while remaining closely connected with the wider clinical team
• Committed to providing compassionate, professional and person-centred support
