About the Role

As our Aged Care Intake & Care Coordinator, you'll be the first point of contact for new participants and their families, guiding them from initial enquiry through to onboarding, while supporting our RN Care Partner with care planning and assessments. You'll work closely with our nursing team, our Business Development Manager, and our multidisciplinary team to make sure every new participant's journey into Pearl's care is smooth, well-documented, and well-supported.

Your role will include:

• Intake & Onboarding: Manage new participant enquiries from first contact through to eligibility screening, service agreements, and onboarding
• Care Plan Support: Prepare intake files and support care plan development and assessments under RN supervision
• MyAgedCare & Referral Liaison: Liaise with MyAgedCare, ACAT assessors and hospital discharge teams on day-to-day intake matters
• Client-Facing Coverage: Provide backup phone coverage for enquiries, representing Pearl professionally and warmly as a client's first impression of our service
• Collaboration: Work closely with our RN Care Partner, casual RN, and BDM to keep the intake pipeline moving and flag opportunities where relevant
• Documentation & Admin: Maintain accurate records, prepare monthly client statements, and support light recurring admin tasks

What We're Looking For

• Experience: 2+ years' experience in intake, case coordination, or care planning within aged care (Support at Home, Home Care Packages, CHSP, or similar)
• Qualifications: Current AHPRA registration as an Enrolled Nurse or Registered Nurse
• Skills: Strong understanding of the Support at Home program and MyAgedCare processes, with sound computer skills to operate care software
• Qualities: Confident owning a process end-to-end, sound judgement on when to escalate, and a warm, professional phone manner
• Certifications: Valid Australian driver's licence, Police Check, and eligibility for NDIS Worker Screening
