Synergie Australia is seeking an Outreach Program Coordinator (APS6) for a trusted client located in Melbourne, VIC.

• Pay: Competitive hourly rate + Super | ⏳ Duration: 12-month contract
• Hours: Monday-Friday | Full-time hours | Flexible working arrangements available

What you will be required to do:

Coordinate community-based providers delivering essential mental health and wellbeing services.

You'll coordinate and manage a national outreach provider program, supporting service delivery professionals, group facilitators, supervisors and referral pathways. This role focuses on ensuring high-quality wellbeing and support services are delivered to clients and their families through a network of external providers.

• Develop and maintain professional relationships with outreach service providers and coordinate program activities.
• Provide onboarding, offboarding, training and ongoing support to service providers.
• Monitor provider performance and contribute to continuous improvement initiatives.
• Review referral requests and assess suitability for program participation.
• Maintain provider databases and communicate availability to internal stakeholders.
• Coordinate treatment programs, workshops and group-based activities to maximise service accessibility.
• Manage complaints and complex issues, ensuring appropriate resolution pathways are followed.
• Act as an escalation point for complex provider and program matters.
• Maintain a small client caseload where required.
• Participate in program evaluation, community engagement and quality improvement initiatives.
• Collaborate with multidisciplinary stakeholders to support effective service delivery.

To be successful in the role you will:

• Hold qualifications as a Psychologist, Occupational Therapist, Registered Nurse, Social Worker or Clinical Counsellor with relevant professional registration.
• Have experience coordinating outreach, community, health, wellbeing or mental health programs.
• Demonstrate exceptional stakeholder management and relationship-building skills.
• Possess strong written and verbal communication skills.
• Have the ability to navigate complex situations and provide practical solutions.
• Demonstrate sound judgement, professionalism and high levels of emotional intelligence.
• Hold, or have the ability to obtain, a Working with Vulnerable People registration or Working with Children Check.
• Be an Australian Citizen and eligible to obtain and maintain the required security clearance.
