Full Time | Ongoing - Rochester & Elmore District Health Service (REDHS)
Rochester & Elmore District Health Service is seeking an enthusiastic and compassionate Lifestyle Team Leader to lead the delivery of engaging lifestyle, wellbeing and social support programs across Yalukang Aged Care.
This is an exciting opportunity for an experienced lifestyle professional who is passionate about enhancing the lives of older people through meaningful activities, community connections and person-centred care.
Working closely with residents, families, volunteers and multidisciplinary teams, you will coordinate innovative lifestyle programs that support independence, wellbeing, identity and social engagement. You will also provide leadership and support to a dedicated team of Lifestyle Assistants while contributing to service development and continuous improvement.
As Lifestyle Team Leader, you will:
Lead, coordinate and evaluate lifestyle and wellbeing programs for aged care residents
Develop personalised lifestyle plans that reflect each person's interests, goals, culture and preferences
Support social, physical, emotional, cognitive, spiritual and community engagement opportunities
Coordinate special events, outings, volunteer programs and community partnerships
Collaborate with Allied Health, Nursing, Social Support and Transition Care Program teams
Provide day-to-day leadership, supervision and mentoring to Lifestyle Assistants
Ensure compliance with the Aged Care Act 2024, Aged Care Quality Standards and REDHS policies
Drive continuous improvement initiatives focused on resident wellbeing and quality of life
To be successful, you will have:
Certificate III or IV in Leisure & Lifestyle, Diversional Therapy or equivalent qualification
Demonstrated experience coordinating lifestyle, recreational or wellbeing programs for older people
Strong understanding of person-centred care and aged care practice
Experience leading and supporting staff in a team environment
Excellent communication, organisational and relationship-building skills
Understanding of dementia-friendly and inclusive engagement approaches
Ability to connect with residents, families, volunteers and community groups
Commitment to creating meaningful experiences that enhance quality of life

Rochester and Elmore District Health Service (REDHS) is situated in Rochester which is the gateway to the mighty Murry River region and close to the regional centres of Echuca, Bendigo and Shepparton.
We are a Small Rural Health service, providing a broad range of acute, residential aged care, preventative and primary health programs to a population of approximately 8,600 people in the Campaspe and Bendigo Shires.
