About Street Side Medics
Street Side Medics is a not-for-profit mobile medical service led by GPs and other health professionals, providing primary healthcare to vulnerable communities and people experiencing homelessness.
Working in partnership with established homelessness charities, shelters and support services, we aim to improve access to healthcare and achieve better health and wellbeing outcomes for people experiencing vulnerability or without stable housing.
About the role
We are seeking an experienced Clinical Nurse Lead – Homeless Healthcare to oversee the clinical coordination and delivery of Street Side Medics' Primary Care Outreach service in Adelaide.
This is a temporary part-time position working two days per week (0.4 FTE) on a two-year contract, commencing in October 2026.
The role is focused on clinical support, coordination of care and the operational delivery of outreach clinics, ensuring safe, effective and continuous care for vulnerable populations.
Working closely with the Clinical Manager, SA Service Coordinator and our volunteer workforce, you will provide hands-on clinical leadership at outreach clinics, support patient care pathways and contribute to ongoing service improvement.
Key responsibilities include:
Acting as a senior clinical presence to lead and support mobile clinics.
Coordinating patient follow-up, results, referrals and ongoing patient management between clinics.
Building relationships with service partners and developing integrated care pathways.
Onboarding, mentoring and supporting volunteer clinicians.
Facilitating pre- and post-clinic huddles and supporting clinic safety and operations.
Contributing to quality improvement, health promotion and clinical governance activities.
Supporting service evaluation, data collection and patient demographic monitoring.
About you
To be successful in this role, you will be an AHPRA Registered Nurse with a current South Australian driver's licence.
Ideally, you will have at least three years' experience in community health, outreach nursing or a similar setting, together with experience in clinical coordination and patient follow-up.
You will have strong communication and interpersonal skills, be confident working within multidisciplinary teams and be comfortable working independently in a mobile or outreach healthcare environment.
Experience supporting vulnerable populations, including people experiencing homelessness, will be highly regarded.
Experience with vaccination services or cold chain management is desirable but not essential.
