About the role
As the Aboriginal and Torres Strait Islander Health Worker Coordinator within the Community Foot Care Hub, you will play a vital role in delivering culturally safe, clinically relevant and person-centred care to Aboriginal and Torres Strait Islander patients with foot disease and high-risk foot conditions. Working as part of a multidisciplinary hub-and-spoke model across Metro North, Central Queensland, Wide Bay and Central West regions, you will support improved health outcomes through care coordination, advocacy, education and culturally informed clinical practice.
This role provides an opportunity to work autonomously within a specialised Aboriginal and Torres Strait Islander health program while collaborating closely with patients, families, communities and healthcare professionals. You will contribute to community engagement, telehealth and virtual care services, health promotion initiatives, quality improvement activities and workforce development, helping to improve access to culturally responsive healthcare and address barriers to care.
