Independent NDIS Support Coordinator - Contractor

We're looking for a Independent Support Coordinator, to join our team and contribute to our mission to build a more inclusive and equitable world for all. If you're an organised multi-tasker with excellent written and verbal skills and a desire to empower people with information and a sense of community.   

The Support Coordinator assists participants to build the skills they need to understand and use their Plan. Also works with the participant to ensure a mix of supports are used to increase their capacity to maintain relationships, manage service delivery tasks, live more independently, and be included in their community.

Qualifications & experience

• Prior Support Coordination or demonstrated experience in Community Services sector or Case Management.

• Extensive knowledge of local disability, mainstream and community services.

• Ability to build NDIS participant and family capacity to understand and navigate service systems

• Demonstrated experience with facilitating multi-disciplinary coordinated planning where multiple providers work together to wrap – around services

• Ability to provide consultation to NDIS participants and their families on the changing NDIS environment

• Demonstrated ability to think creatively and problem solve

• Strong written and computer skills for the recording of notes, gathering evidence, written reports and support planning.
