NDIS Support Coordinator

About the role

As a Support Coordinator, you will work alongside NDIS participants to understand their goals, coordinate appropriate supports and build their capacity to navigate the NDIS and broader community systems.

Key responsibilities

• Coordinating and monitoring participant supports and services
• Supporting participants to understand and exercise their NDIS rights
• Advocating for participant choice, safety, wellbeing and quality of life
• Building participant capacity and independence
• Developing and maintaining effective support networks
• Working collaboratively with families, support providers, allied health professionals and other stakeholders
• Identifying and addressing barriers to service access
• Supporting participants through complex or changing circumstances
• Maintaining accurate and professional documentation and case notes
• Ensuring services are delivered in accordance with NDIS requirements and best-practice principles

About you

• Previous experience in NDIS Support Coordination, disability services, community services, case management or a transferable role
• A strong understanding of the NDIS and broader disability/community service systems
• Experience supporting people with complex needs and/or navigating safeguarding concerns
• Excellent communication and interpersonal skills
• Strong organisational, time-management and documentation skills
• The ability to build respectful and professional relationships with participants and stakeholders
• A strong understanding of confidentiality, professional boundaries and ethical practice
• The ability to work autonomously while contributing positively to a team
• A current Australian driver's licence and access to a comprehensively insured, registered and roadworthy vehicle are essential
• Relevant qualifications in Community Services, Disability, Social Work, Nursing, Mental Health or a related discipline are desirable
