Disability Support Worker

Support Worker – Baldivis WA and Midland WA

About the Role

Headway Support Partners is looking for compassionate, reliable and experienced Support Workers to join our team in Baldivis and Midland

You will provide person-centred support to a participant in their home and community, helping them maintain their independence, wellbeing and quality of life.

We are looking for people who are caring, patient and confident supporting participants with a range of daily living and support needs.

Key Responsibilities

• Provide high-quality, person-centred support based on the participant's individual needs.
• Assist with personal care, daily living and household activities where required.
• Support community access, social activities and appointments.
• Encourage independence and choice.
• Follow individual support plans and Behaviour Support Plans where applicable.
• Support medication routines in line with the participant's plan and organisational procedures.
• Communicate effectively with families, Support Coordinators and other professionals.
• Complete accurate progress notes, incident reports and other required documentation.
• Maintain professional boundaries, privacy and confidentiality.
• Promote a safe, respectful and inclusive environment.

Requirements

• Certificate III or IV in Individual Support, Disability or equivalent preferred.
• Previous experience working in disability or community support.
• NDIS Worker Screening Check.
• Working with Children Check.
• Current First Aid and CPR.
• Valid Australian driver's licence and reliable vehicle preferred.
• Good communication and interpersonal skills.
• Reliable, punctual and professional.
• Ability to work independently and as part of a team.
• A genuine commitment to supporting people with disability.

Experience with complex support needs, positive behaviour support, manual handling, or other specialised supports will be highly regarded where relevant to the participant.
