Disability Support Worker

Location: Alexandra Hills and surrounding areas
Employment Type: Casual
Hours: Approximately 20–40 hours per fortnight
Pay Rate: SCHADS Home Care Employee classification – Starting at $36.08

About the Role

Lifest HealthCare is seeking an experienced Support Worker to provide ongoing support to participant in Alexandra Hills and surrounding areas.

This role is primarily focused on supporting participants with mental health and psychosocial support needs, with an emphasis on building independence, maintaining routines and supporting meaningful participation within the community.

The role includes community access, attending medical and allied health appointments, social activities, daily living support and emotional support, along with accurate documentation and reporting.

Current Shift Times

This position operates across a Monday to Sunday roster, with current shifts including:

• Afternoon shifts – approximately 1:00pm to 5:00pm

Key Responsibilities

• Provide person-centred support to participants with mental health and psychosocial support needs
• Support participants to access the community and participate in social and recreational activities
• Assist participants to attend medical, allied health and other appointments
• Provide emotional support and encouragement while maintaining professional boundaries
• Support participants to develop and maintain daily routines
• Encourage independence, choice and participation in everyday activities
• Assist with domestic tasks where required
• Support participants to work towards their individual goals
• Complete detailed shift notes and participant documentation
• Complete incident reports and other documentation where required
• Follow participant support plans, routines and organisational procedures
• Communicate effectively with participants, families and the wider support team

About You

We are looking for someone with a calm, mature and professional approach who is confident supporting people experiencing mental health and psychosocial challenges.

You will be comfortable working independently, building positive and respectful relationships with participants and maintaining appropriate professional boundaries.

Strong communication skills are important, particularly the ability to listen, provide reassurance and adapt your approach to the individual needs of each participant.

Strong written communication skills are also required, as the role involves regular completion of shift notes and other participant documentation.

Previous experience supporting people with mental health or psychosocial disabilities will be highly regarded.

Key Requirements

• Certificate III or IV in Individual Support, Disability, Mental Health or similar is desirable
• Current NDIS Paid Worker Screening Card, or willingness to obtain
• Current First Aid and CPR certificates, or willingness to obtain
• Current driver's licence
• Reliable and insured vehicle
• Strong written and verbal communication skills
• Ability to work independently and follow established support plans
• Confidence supporting participants within the community and attending appointments
