The ATSICHS Vulnerable Youth Service team provides tailored case management support for young Aboriginal and Torres Strait Islander peoples. The team incorporates various services and funding streams, all focused on vulnerable young people. The team incorporates various services and funding streams, all focused on vulnerable young people. The ‘Vulnerable Youth (Youth Justice, CYRD)’ role is funded through the Community Youth Response Diversionary (CYRD) Intensive Case Management Program. The role supports high risk/complex needs young people, at risk of re-offending or entering the Youth Justice system. This is achieved by a strong assertive outreach focus and delivering tailored holistic supports to young people. The model incorporates a family connection approach for delivering targeted supports and interventions, working also with immediate/extended family to support the young person.

Duties

• Effectively engage clients through cold-call home-based visits. Turn initial ‘service offerings’ home visit into consent by delivering motivational interviewing and positive reinforcement practice interventions.  
• Deliver immediate safety assessment and safety responses, documented and approved by the line manager, in instances of domestic and family violence, self-harm/mental health, and child abuse.
• Facilitate ATSICHS ‘Narrative Assessment Tools’ completed in full by client ensuring the voices of clients are imbedded in their assessment. Facilitate conversations and introspection on all assessment domains. 
• Deliver Empowerment Interventions that Build Client Belief, Motivation, and Self-Sufficiency.
• Deliver case managed services according to the program model of care, ensuring minimum case load requirements are maintained to ensure equitable access to services for community.
• Maintain accurate and detailed case notes of all client interactions, ensuring case notes are written from the perspective of clients in their voice.
• Complete SRS (client-led brief session rating survey) upon each occasion of direct face-to-face support provided to client.
• As per MOBnet Performance module, participate in monthly prescheduled supervision with direct line Manager. Ensure Supervision tasks are the priority in work completion. Ensure all associated monthly supervision tasks in MOBnet are completed (two-way feedback, team culture score, etc.).

• Base Salary of $85,214 per annum (Based on qualification and experience).

• Limited/No Qualifications + Moderate/Low Work History: Less than 8 years working full-time in another industry and a commitment to complete Diploma (funded by ATSICHS) while working.

OR 

• Limited/No Qualifications + Strong Work History with Transferrable Skills: At least 8 years working full-time in another industry and a commitment to complete Diploma (funded by ATSICHS) while working.

AND

• Sound case management skills, including grammatical writing ability, personal organisation and time management, and execution of services according to the model of care phases of work. 
• Demonstrated ability to understand and apply social work theoretical frameworks of practice, aligned with ATSICHS values.
• Ability to deliver other staff on client interventions that build client motivation, insight and acceptance, self-awareness, decision making (agency), and sustainable behaviour change.
• Ability to apply technical knowledge of relevant legislation (child protection/housing/youth justice etc).
• Ability to critique and problem solve legislative grey zones, while managing organisational and child/client harm risks.
• Ability to proactively engage and develop strong working relationships with decisions of external stakeholders, including Child Safety, Youth Justice, Schools, and Hospitals.  
• Ability to identify through systems panel coordination numerous internal ATSICHS programs and services to activate wrap-around care for clients, trading-off and negotiating resources across the organisation and between other teams to achieve client impact. 

Additional Requirements

• National Police Certificate.
• Current Working with Children's Card (Blue Card) or be eligible to apply.
• Driver's Licence.
• Evidence of Work Rights.
• Travel to work in the community on a regular basis.
