Community Care – Clinical Care Coordinator

We are seeking a clinical care coordinator for the Townsville region. Supporting our clients to live and age well in their own home and be active and participate in their community is our focus.  We have established some innovative and exciting opportunities for our clients.

Our clients receive services in their individual home environments, this also includes residents in retirement villages.  We have established relationships with several retirement village partner sites across the region and part of your role will be representing Envigor weekly at the partner sites.  You will provide regular information sessions to prospective clients and be a point of connection for our existing clients. 

To be successful in this role you will hold; 

• Bachelor’s degree in nursing, Current registration with APHRA and evidence of recent proactive professional development

• Demonstrated experience in coordinating or assisting in the care coordination of a community-based service or client care.

• Knowledge and understanding of relevant Aged Care and Disability legislative frameworks, standards and funding requirements or ability to quickly acquire such knowledge.

• Proficiency in Microsoft Office suite 

• Current Yellow Card or National Disability Worker Screening Clearance - or -

• Current National Police Check  

• If you have been a citizen in another country after the age of 16 you will also need to have a statutory declaration as per the "Police Certificate Guidelines for Aged Care Providers" 

• Current Influenza Vaccination Certificate 

• CPR certificate 

• Current Driver's Licence with own fully insured vehicle 

• COVID-19 (fully vaccinated)
