Northern Health are seeking applications from motivated and suitably experienced Diabetes Nurse Educator to join the Ambulatory Diabetes team - Hospital Admission Risk Program (HARP).  This is a long service cover from 7 September to 16 October 2026.

Northern Health is the major provider of acute, maternity, sub-acute, mental health, specialist, community and home-based services in Melbourne’s rapidly growing outer north.

Services are provided through our six main campuses:

Northern Hospital Epping, Broadmeadows Hospital, Bundoora Centre, Craigieburn and Mernda Community Hospitals Centres and Kilmore District Hospital.  In addition, Mental Health Services are provided at Epping, Broadmeadows, Jacana, Preston, Mill Park, Mernda and Coburg.

About the Role:

The Diabetes Nurse Educator provides comprehensive, patient-centred care to individuals with diabetes and their carers, and education of team members about diabetes management within the Hospital Admission Risk Program (HARP). 

The role will require high level expertise in diabetes, focussing on managing individuals with complex unstable diabetes at high risk of hospital admission. It will also encompass direct education and support for women with diabetes in pregnancy.

This position is 32 hours per week (0.8EFT), based at Craigieburn Community Hospital working within a multidisciplinary diabetes team including Endocrinologists, Diabetes Nurse Educators, allied health clinicians and other relevant operational staff to deliver a high-quality diabetes service.

To be successful in this role you will have:

• Current Australian Diabetes Educators Association credentialing status or working towards this.
• A minimum of 12 months of clinical experience in diabetes management.

• Experience and good working knowledge of Type 1, Type 2, Type 3c diabetes management. Assess, plan and implement a suitable education program, individualised to suit the client with diabetes, including the family and significant others, where possible.
• Experience and knowledge in managing women with diabetes in pregnancy.

• Familiar with the use and application of diabetes technology such as Continuous Glucose Monitor as well as insulin pumps.
• Proven ability to work autonomously and in a team-based environment, with a high level of problem solving and planning ability.
• Experience working with clients from culturally and linguistically diverse backgrounds.
• Current Victoria Drivers License
