Your Role: As a Registered Practice Nurse, your responsibilities will include:

• Conducting thorough patient assessments and diagnoses.

• Prescribing medications and recommending lifestyle changes to effectively manage chronic conditions.

• Ordering and analyzing pathology and radiological tests.

• Educating and supporting patients with chronic diseases through tailored Individual Care Plans.

• Promoting health literacy and patient engagement within the community.
