We are committed to empowering participants to achieve their goals through tailored, culturally responsive, and outcome-focused services.As part of our continued growth, we are seeking an experienced Support Coordinator to join our Western Australia and Victoria teams based in Malaga and Werribee respectively.This is a flexible part-time/ Contractor role, with the potential to transition to full-time as the caseload grows.About the RoleYou will work closely with NDIS participants to implement their plans, coordinate supports, and build their capacity to navigate the NDIS independently.
Key responsibilities include:Supporting participants to implement their NDIS plans in line with their goals, funding, and preferencesCoordinating and connecting participants with appropriate providers, services, and community supportsMonitoring service delivery, budgets, and outcomes to ensure supports remain effective and sustainableMaintaining accurate case notes and documentation in line with NDIS and organisational requirementsPreparing progress reports and review documentation for reassessments and plan variationsLiaising with families, carers, providers, NDIA representatives, and other stakeholdersIdentifying risks, service gaps, or safeguarding concerns and escalating appropriatelyPromoting participant capacity building, choice, and control in all decision-makingThis role requires someone who is confident in managing a caseload independently, has strong attention to detail, and maintains professional boundaries and compliance standards.About YouWe are looking for a proactive, organised professional who is passionate about advocating for participants and improving outcomes.Essential Requirements:Demonstrated experience working in the NDIS sectorStrong understanding of NDIS processes, funding categories, and plan implementationAbility to manage a caseload autonomouslyExcellent written and verbal communication skillsCurrent Driver’s Licence and reliable vehicleNDIS Worker Screening CheckWorking With Children CheckFull working rights in Australia (Citizen, Permanent Resident, or valid visa with work rights)Desirable:Qualification in Social Work, Community Services, Psychology, Allied Health, or related fieldExperience supporting participants with complex needsExperience in Local Area Coordination (LAC)Experience in Case ManagementWhat We Offer20 hours per week with flexible part-time arrangementsPotential to transition into a full-time positionSupportive leadership and strong administrative backingOpportunity to grow with an expanding national providerCompetitive pay aligned with experience and the SCHADS AwardA collaborative, values-driven team culture
