Lead organisation-wide change and make a meaningful difference to the care of patients living with cognitive impairment.
Cabrini is seeking an experienced Project Coordinator - Cognitive Care to lead an organisation-wide program focused on improving the prevention, recognition and management of delirium, dementia, cognitive impairment and behavioural distress.
Working collaboratively with Nursing, Allied Health, Medical, Patient Safety and Education teams, you will translate evidence into practice and embed sustainable clinical systems, education and governance that improve outcomes for patients, families and staff.
This is a part-time 12 months fixed term role.
