Picture this: you're reviewing George's care plan and he mentions he's been missing medical appointments since he stopped driving. Together, you reorganise his services to include transport and a support worker to help him navigate those confusing specialist visits. The relief on his face? That's what being a Care Partner is all about - working with our seniors to give them the best support possible.

At Murray Mallee Aged Care Group (MMACG), we know that behind every great client outcome is an even greater Care Partner: the person who turns a list of services into meaningful moments. Based in our Murray Bridge office, you'll be a champion of client support, helping seniors across our community live with dignity and independence.

• Conduct Client Assessments – Visit clients in their homes to understand their unique needs and goals.

• Create Personalised Care Plans – Design service packages that fit like a glove.

• Review Client Progress – Read support worker notes like a detective, spotting opportunities to build independence.

• Organise Services – Coordinate everything from meal deliveries to mobility aids, making sure each piece fits the bigger picture.

• Manage Equipment Purchases – Help clients get what they need, from grab rails to bathmats.

• Run Regular Plan Reviews – Keep care plans fresh and relevant as clients' needs change.

• Certificate III in Community Services (or equivalent experience, with a willingness to learn).

• Current SA driver's licence and reliable vehicle.

• National Police Certificate (or willingness to obtain one).

• Understanding of care or community services.

• Strong customer-service skills.

• Cultural awareness and sensitivity.
