Assessment Officer

In this role, you’ll complete holistic assessments and reviews for clients under the Home and Community Care Program for Younger People (HACC-PYP) and other funded services. You may also provide community nursing support as required.

This opportunity will suit an experienced Registered Nurse, Community Nurse Assessor, RAS or ACAS assessor who enjoys working directly with clients and understanding their care, psychosocial and functional needs.
