Support Workers – Disability & Community Services

About the role

Synergy Community Services Pty Ltd is seeking compassionate, reliable and motivated Disability Support Workers to join our growing team. You will provide person-centred support to NDIS participants in their homes and the community, assisting with daily living, personal care, community participation and independence.

Key responsibilities

• Personal care, showering, dressing and grooming

• Assistance with daily living and household activities

• Meal preparation and domestic assistance

• Community access, appointments and social activities

• Mobility and transfer support

• Supporting participant independence and choice

• Maintaining accurate shift/progress notes

• Reporting incidents, hazards and changes in participant needs

• Following NDIS requirements, participant support plans and organisational policies

About you

• Current Australian Driver's Licence – Mandatory

• Reliable, registered and comprehensively insured vehicle – Mandatory

• NDIS Worker Screening Clearance

• First Aid & CPR certificates

• Australian working rights

• Ability to complete required background and reference checks

• Good communication and documentation skills

• Commitment to participant rights, dignity, privacy and choice

• Ability to work independently and as part of a team

• Certificate III/IV in Individual Support, Disability, Community Services or related field (desirable)
